I think in part it’s due to them needed to live up to Johns status as “badass cowboy of the 2010s” so they did that and more and they didn’t want you playing as a Bill type so we got another John type character design and personality wise with the gritty sort of outlook but a good personality and a soft spot along with that Arthur is one of the two characters in the entire rdr series

Yeah Arthur's being shot in the chest, besten,, starved, and drowned probably didn't help But my head cannon is it's much more likely he got it from Hosea, who was actively taking steps to mitigate symptoms Who in turn likely got it during the Civil War But we also don't have a solid timeline for how long the gang is anywhere at all
